Placement of fields on edit form different than add form

Another usability issue: when you are creating a new ticket, the fields have one orientation and when you save the ticket and go to edit it, the same fields have a different orientation. This is very confusing from a user interface perspective. I found myself entering the values in the wrong field because my eyes had expected the field to be in the same place as it was on the previous form.

If possible, we should look into how to modify these forms so that the fields are always in the same place, rather than confusing the user by moving them to different locations.
